This study gives you two decision rules for clinic. First, when a patient over 66 presents with unilateral shoulder pain and a full-thickness tear, image the contralateral shoulder. There is a coin-flip chance of bilateral disease, and 35.5% will have a full-thickness tear on the other side that may become symptomatic over time. Second, when managing a known tear nonoperatively, follow it with yearly ultrasound. Tear enlargement is the most likely trigger for new pain onset.
The 30% size difference between symptomatic and asymptomatic tears in the same patient is the central finding.
