The 2013 IDSA guidelines provide consensus recommendations for diagnosing and managing prosthetic joint infection. They cover the full surgical decision tree — debridement and retention, one-stage exchange, two-stage exchange, resection, and amputation — with organism-specific antibiotic protocols. Most recommendations are expert opinion-based given limited RCT data in this field.

Key Findings

Debridement and retention is appropriate only when all three criteria are met:
- Well-fixed prosthesis
- No sinus tract present
- Within 30 days of implantation OR 3 weeks of symptom onset
- Outside these windows, relapse is significantly more likely (B-III).
Staphylococcal PJI after debridement and retention requires rifampin combination therapy:
- 3 months total for THA
- 6 months total for TKA
- First-line companions: ciprofloxacin (A-I) or levofloxacin (A-II).
- The longer TKA duration reflects greater biofilm burden and treatment difficulty in the knee.
Rifampin must never be used as monotherapy — resistant mutants emerge rapidly under single-drug pressure, which is why a companion drug is always required.
Sonication of the explanted prosthesis outperforms single tissue culture for PJI diagnosis:

- Single tissue culture sensitivity: 60.8% (P <.001)
- Send at least 5-6 periprosthetic specimens. Fewer than 5 reduces culture sensitivity.
Surgical success rates by strategy:
- Two-stage exchange: 87% (dominant US approach for chronic PJI)
- One-stage exchange: 80%-90% for THA (requires known susceptible pathogen, good soft tissue, pathogen susceptible to highly bioavailable oral agents)

Board PearlDebridement and retention for PJI requires a well-fixed implant, no sinus tract, and presentation within 30 days of surgery or 3 weeks of symptoms — outside this window, two-stage exchange is preferred.
