When a professional athlete sprains their ankle on the field, resist the urge to make a definitive diagnosis at pitch-side. The initial exam is useful only as a triage tool. Send them back in 4–5 days for a proper delayed examination — that is when localized tenderness, haematoma, and the anterior drawer test become diagnostically meaningful.
For the general population, grade II and III injuries are managed non-operatively: RICE for the first 4–5 days, short immobilization in a boot or cast for 5–10 days, then a structured functional program with a lace-up brace. Therapeutic ultrasound and electrical stimulation add nothing and should not be used.
For the professional athlete, the calculus shifts. Objective instability after non-operative treatment predicts future sprains, and athletes face higher demands than the general population. Acute anatomical repair in this cohort reduces long-term instability and should be performed at a high-volume center by an experienced surgeon.
