When a competitive pitcher presents with medial elbow pain and UCL insufficiency, the first conversation is almost always about surgery. This study quantifies what nonoperative treatment actually delivers: a 42% chance of returning to the prior level of play at roughly 6 months, with no way to predict in advance who will make it.
That 42% number is the critical counseling anchor. It means nonoperative care is a reasonable first attempt for a motivated athlete willing to accept better-than-even odds of ultimately needing surgery. It is not a strategy with a high probability of success, and framing it that way to patients matters.
For diagnosis, resist over-relying on MRI. A 57% sensitivity means a normal MRI does not rule out UCL insufficiency. The milking sign, valgus stress pain, and tenderness directly over the anterior UCL bundle are your primary diagnostic tools.
For boards: know that the anterior bundle of the UCL complex is the primary valgus stabilizer, that peak valgus stress occurs in the late cocking and acceleration phases, and that nonoperative care carries a 42% return-to-sport rate vs. Roughly 68–79% with reconstruction.
