When exposing the anteromedial hip capsule via a Smith-Petersen approach — whether for periacetabular osteotomy, open reduction, or anterior total hip arthroplasty — expect to encounter the iliocapsularis as a distinct muscle belly between the iliacus and rectus femoris; it must be elevated sharply from the capsule to develop the capsule–psoas tendon interval, and rough handling risks heterotopic ossification.
