This retrospective longitudinal cohort study followed 45 patients with a symptomatic shoulder and a sonographically confirmed asymptomatic full-thickness tear on the contralateral side. Over a mean follow-up of 2.8 years (range 3–8 years), patients completed ASES-based questionnaires and 23 returned for repeat bilateral ultrasound. The study asked: how often do asymptomatic tears develop symptoms, and do they enlarge over time?
When you find an incidental full-thickness rotator cuff tear on the contralateral side of a patient presenting with shoulder pain, counsel them directly: roughly half will develop symptoms within 3 years, and the tear will not get smaller on its own.
This paper is the foundation for why we don't simply reassure patients with asymptomatic tears and discharge them. Initial conservative management is appropriate, but patient education and follow-up imaging are essential — not optional.
Two findings especially shape surgical decision-making: the development of night pain and loss of active elevation in newly symptomatic patients both correlate with failure of conservative care in prior literature, suggesting these are your triggers to escalate management.
One important caveat: this cohort was specifically patients with a contralateral symptomatic tear, who likely represent a biologically higher-risk group. The 51% progression rate probably overestimates risk in the general population with truly isolated asymptomatic tears.
