This study asked whether augmenting large-to-massive rotator cuff repairs with the Restore Orthobiologic Implant, a porcine small intestine submucosa xenograft, improves strength, function, or retear rates. When a biologic scaffold is marketed for cuff augmentation, ask what species the safety data came from before you trust it. Restore looked promising in dog studies, but dogs do not mount the immune reaction humans do. In this human series roughly one in five patients had a severe inflammatory reaction that mimicked infection, cultured negative, and required a return to the OR for graft removal.
A negative-culture painful swollen shoulder weeks after a xenograft repair should raise immune reaction to the graft, not just deep infection.
Beyond the reaction, the graft did nothing useful: no lower retear rate, no added tendon thickness, and actually worse internal rotation and lift-off strength. The lesson for cuff surgery is that a scaffold has to be both immunologically safe in humans and mechanically helpful, and this one was neither.
