• Malrotation is an underrecognized but functionally significant complication of femoral nailing, and this study quantifies how technique choice affects its incidence — a nearly fourfold difference in rates of clinically relevant malrotation. • The findings suggest that manual traction, without a fracture table, can shorten operative time without requiring more assistants or increasing fluoroscopy exposure. • For residents learning femoral nailing, this study provides a direct, randomized comparison of two setup techniques that are otherwise chosen largely by surgeon preference or institutional habit.
