• For both-column acetabular fractures, this study provides comparative data suggesting the AIP approach may offer an advantage in achieving anatomic reduction over the traditional ilioinguinal approach, an important consideration when selecting an approach for complex posterior column/quadrilateral plate involvement. • The proposed anatomic rationale — contralateral access to the posterior column and quadrilateral plate, and the ability to place a buttress plate below the pelvic brim — gives trainees a framework for why AIP exposure may facilitate reduction in select fracture patterns. • As a retrospective, Level III comparison across two non-overlapping time periods, the findings should be interpreted as supportive rather than definitive evidence for approach selection.
