This case series introduces the belly-off sign, a new physical exam maneuver for subscapularis lesions. The arm is passively placed in flexion and maximal internal rotation with the palm on the abdomen, then the patient holds the position after the wrist is released. The authors compared it against the lift-off test, internal rotation lag sign, and Napoleon sign across seven types of subscapularis lesions.
