Commonly used return-to-sport criteria after ACL reconstruction have never been validated against actual re-rupture outcomes. This prospective cohort followed 158 male professional athletes to test whether a battery of objective strength and functional discharge criteria predicts ACL graft rupture after they return to competition.
Clear an athlete to return only after they pass every objective discharge criterion, not most of them. Athletes who returned without meeting all six criteria had four times the graft rupture risk, and one in three of them tore the graft.
Rebuild the hamstrings, not just the quadriceps. A lower hamstring-to-quadriceps ratio independently predicted rupture, because the hamstrings resist the anterior tibial translation that stresses the graft. Respect the early window. About two-thirds of ruptures happened within 6 months of return, which fits the dogma against returning before 6 months.
