This is a randomized controlled trial of open repair of chronic two-tendon (supraspinatus and infraspinatus) rotator cuff tears. It asks whether augmenting the repair with a porcine small intestine submucosa patch (Restore) improves tendon healing and clinical scores. Healing was assessed by MR arthrogram at one year.
When you are tempted to reach for a biologic patch to salvage a massive cuff repair, this trial should give you pause. Restore porcine small intestine submucosa did not improve healing and trended toward worse function scores, with a sterile inflammatory reaction in 3 of 15 patients.
The mechanistic reason matters: the scaffold is biologic, not structural. It resorbs and loses strength in the first few weeks, exactly when repairs fail mechanically. A biologic booster cannot overcome a poor mechanical environment.
The more durable lesson is that tear size drives everything. Large tears healed 89% of the time, massive tears only 24%. This trial also cleanly demonstrated the anatomic-clinical link: a healed cuff means better pain, function, and satisfaction.
For boards, remember Restore is an acellular extracellular matrix scaffold and this RCT is the reason it is not recommended for massive cuff augmentation.
