When evaluating a young patient with corticosteroid-induced osteonecrosis at Steinberg stage I or II, concentrated bone marrow MSC injection at the time of core decompression offers a dramatically lower risk of collapse, THA, and — critically — THA revision compared with decompression alone; the revision burden is where these patients' quality of life deteriorates over decades, making early cell therapy a decision with 25-year consequences.
