It asked which tears go on to develop and progress fatty muscle degeneration, and how quickly that happens after a tear enlarges. The study used bilateral cuff-tear patients (asymptomatic side studied, painful contralateral side excluded from analysis).
When you follow an asymptomatic or minimally symptomatic degenerative cuff tear, tear enlargement is the signal that matters. A tear that grows, especially in width, is far more likely to develop progressive fatty muscle degeneration than a stable tear, and that risk holds independent of age.
Watch the tears already larger at baseline (around 15 mm) and those enlarging by about 1 cm. These are the ones heading toward muscle loss. The timeline is short. Once a tear enlarges, fatty degeneration often progresses within about a year, and because fatty change does not reverse after repair, waiting can cost you a repairable muscle.
Don't over-weight anterior cable disruption on its own. This study shows that association is mostly a proxy for a bigger tear once you adjust for size. For a painful tear with these features, consider active surveillance with serial imaging or earlier surgical intervention.
