Diagnosing periprosthetic infection after shoulder arthroplasty is hard because indolent organisms like P acnes blunt standard tests. This study measured 9 synovial fluid cytokines in 75 revision cases to test whether a cytokine panel could diagnose infection better than serum ESR and CRP. It asked whether combining multiple cytokines outperforms any single marker.
When a patient presents with a painful shoulder arthroplasty and only pain and stiffness, you cannot rely on ESR and CRP. Their sensitivity in the shoulder falls below 50% because P acnes and coagulase-negative Staph provoke a weak systemic response.
This paper shows synovial fluid cytokines sample the response directly in the joint. IL-6 alone reaches an AUC of 0.87, and a combined IL-6, TNF-α, IL-2 model reaches specificity 0.93 with a likelihood ratio positive of 12.0.
The practical mental model: negative serum markers do not rule out shoulder PJI. A cytokine-based probability could tip a borderline case toward 2-stage exchange or an early IV antibiotic course after an unexpected positive culture.
Weight the evidence appropriately. This is a Level III single-center diagnostic study with no gold standard for shoulder PJI, only 8 probable-contaminant cases, and the model still needs prospective validation before routine use.
