When counseling a patient with a moderate-to-severe bunion, lead with genetics: bilateral involvement and a family history are the rule (both ~84%), not the exception, and deformity severity is independent of Achilles tightness, flatfoot, or first ray hypermobility.
Don't miss concomitant lateral metatarsalgia — nearly half of these patients have it, but most won't have callosities to tip you off.
