When you see any cruciate tear, actively examine for PLC involvement with varus stress testing and the dial test — missing a concurrent grade III PLC injury will overload your cruciate graft and cause early failure.
If laxity is confirmed, reconstruct (don't repair) within 3 weeks for best outcomes, and in chronic cases correct varus malalignment before touching the ligaments.
