When evaluating an elderly patient for elective TKA, flag congestive heart failure, renal disease, and metastatic cancer as the highest-risk conditions for perioperative mortality, and counsel patients with CHF, preoperative anemia, depression, chronic pulmonary disease, or diabetes that their PJI risk is meaningfully elevated — these findings support targeted preoperative medical optimization before proceeding.
