Multicenter retrospective case series evaluating mid- to long-term clinical and radiographic outcomes of bony increased-offset RSA (BIO-RSA) — autologous humeral head graft placed under the glenoid baseplate to lateralize the center of rotation — in 143 shoulders with rotator cuff deficiency followed for a minimum of 5 years (mean 75 months).
When planning BIO-RSA, target an RSA angle <5° and at least 5 mm of glenosphere inferior overhang — favor a 25-mm baseplate with a 36-mm sphere or a 29-mm baseplate with a 42-mm sphere, and consider a less medialized humeral component (135° or 145°) over the classic 155° Grammont design to reduce notching risk, particularly in thin patients with low BMI.
