When choosing a postoperative ACL protocol, you can safely favor accelerated rehabilitation after a BPTB reconstruction. Compressing the same exercises into 19 weeks instead of 32 produced no extra anterior knee laxity, no worse function, and no worse cartilage response at 2 years.
The mechanistic lesson is that quadriceps-dominated exercise near full extension and early weightbearing do strain the graft, yet giving that strain earlier did not stretch the graft out any more than delaying it.
One practical caveat: this protocol was deliberately conservative, starting vigorous quadriceps work at the end of week 1 because earlier attempts caused pain and swelling. Do not extrapolate to immediate, unrestricted vigorous quadriceps loading.
The sobering finding is that cartilage biomarkers stay abnormal well past 2 years, long after patients return to sport. Rehab timing does not fix this, which reinforces counseling patients that reconstruction restores stability but does not eliminate long-term OA risk.
