When counseling patients on ACL graft choice, BPTB provides objectively superior laxity control and pivot shift grades at 3 years, but the functional and satisfaction gap is smaller than the mechanical numbers suggest — and this study's hamstring construct (two-strand, extra-articular fixation) is not equivalent to the four-strand intraosseous grafts used today, so these laxity differences may not fully translate to modern hamstring technique.
