Complications of Anterior Cervical Spine Surgery: a Systematic Review of the Literature

Key Findings

  • Dysphagia is underestimated by retrospective data: the pooled rate is 5.3% overall, but prospective capture yields 11.5%. Chronic dysphagia (>3 months) shows an even wider gap: 10.1% prospectively vs. 0.8% retrospectively. BMP use and multilevel surgery are the most consistent risk factors.
  • Adjacent segment disease affects 8.1% overall, but the number that matters clinically is 4.5% — cases requiring reoperation. With mean time to reoperation approximately 3 years. Shorter fusions paradoxically carry higher ASD risk than longer constructs, likely because at-risk adjacent segments are incorporated into longer fusions.
  • C5 palsy occurs in 3.0% and presents at a mean of 1.7 days postoperatively, not in the recovery room. The mechanism is stretch-induced ischemia. Preoperative spinal cord rotation ≥10° is an independent predictor (mean 10.3° in affected patients vs. 2.1° in unaffected patients).
  • Cervical hematoma is rare at 1.0%, but drives reoperation in 46.1% of affected patients. The high re-exploration rate reflects the risk of rapid airway compromise or neurologic deficit. Independent predictors include multilevel ACDF, INR >1.2, BMI <24 kg/m², and ASA class ≥3.
  • Esophageal perforation carries mortality up to 33% despite a pooled incidence of only 0.19%. Presentation can be delayed by up to 20 years. Survivors resume oral intake in 60–100% of cases. Management is nearly always aggressive: broad-spectrum antibiotics, debridement, hardware removal, and primary esophageal repair.
  • RLN palsy occurs in 1.3% overall, with partial or complete recovery in 83.4% of patients. Revision surgery substantially increases risk: 8% in secondary vs. 2% in primary procedures, likely due to disrupted fascial planes.
  • CSF leak risk increases dramatically with OPLL (relative risk 13.74) and revision surgery (relative risk 2.75) compared to primary ACDF without OPLL. The vertebral artery lies within 5 mm of the uncinate process tip. Aberrant medial location can cause injury even with standard landmarks, making preoperative imaging review essential.
Board PearlProspective data captures 11.5% dysphagia and 10.1% chronic dysphagia after anterior cervical surgery — retrospective series miss most cases.

Clinical Relevance

The complication rates from retrospective series — historically the dominant source in consent discussions. Substantially undercount dysphagia. When counseling patients, use prospective benchmarks: approximately 11% will have some dysphagia postoperatively, and roughly 10% may have symptoms lasting beyond 3 months.

When you see a patient on postoperative day 1–2 after ACDF with new deltoid weakness, think C5 palsy before thinking hardware failure or hematoma. It characteristically appears at a mean of 1.7 days. Check preoperative imaging for cord rotation. Rotation ≥10° flags elevated risk.

Cervical hematoma demands a low threshold for return to the OR. Nearly half of all hematomas require surgical evacuation, and the window before airway compromise narrows quickly. Flag your highest-risk patients preoperatively: multilevel surgery, INR >1.2, BMI <24, and ASA ≥3 are independent predictors.

Esophageal perforation can present years or decades after surgery with sepsis, aspiration pneumonia, or even meningitis. When a patient with remote anterior cervical surgery presents with any of these findings, esophageal perforation belongs on the differential regardless of time elapsed since the index operation.

Key Findings

  • Dysphagia is underestimated by retrospective data: the pooled rate is 5.3% overall, but prospective capture yields 11.5%. Chronic dysphagia (>3 months) shows an even wider gap: 10.1% prospectively vs. 0.8% retrospectively. BMP use and multilevel surgery are the most consistent risk factors.
  • Adjacent segment disease affects 8.1% overall, but the number that matters clinically is 4.5% — cases requiring reoperation. With mean time to reoperation approximately 3 years. Shorter fusions paradoxically carry higher ASD risk than longer constructs, likely because at-risk adjacent segments are incorporated into longer fusions.
  • C5 palsy occurs in 3.0% and presents at a mean of 1.7 days postoperatively, not in the recovery room. The mechanism is stretch-induced ischemia. Preoperative spinal cord rotation ≥10° is an independent predictor (mean 10.3° in affected patients vs. 2.1° in unaffected patients).
  • Cervical hematoma is rare at 1.0%, but drives reoperation in 46.1% of affected patients. The high re-exploration rate reflects the risk of rapid airway compromise or neurologic deficit. Independent predictors include multilevel ACDF, INR >1.2, BMI <24 kg/m², and ASA class ≥3.
  • Esophageal perforation carries mortality up to 33% despite a pooled incidence of only 0.19%. Presentation can be delayed by up to 20 years. Survivors resume oral intake in 60–100% of cases. Management is nearly always aggressive: broad-spectrum antibiotics, debridement, hardware removal, and primary esophageal repair.
  • RLN palsy occurs in 1.3% overall, with partial or complete recovery in 83.4% of patients. Revision surgery substantially increases risk: 8% in secondary vs. 2% in primary procedures, likely due to disrupted fascial planes.
  • CSF leak risk increases dramatically with OPLL (relative risk 13.74) and revision surgery (relative risk 2.75) compared to primary ACDF without OPLL. The vertebral artery lies within 5 mm of the uncinate process tip. Aberrant medial location can cause injury even with standard landmarks, making preoperative imaging review essential.

Clinical Relevance

The complication rates from retrospective series — historically the dominant source in consent discussions. Substantially undercount dysphagia. When counseling patients, use prospective benchmarks: approximately 11% will have some dysphagia postoperatively, and roughly 10% may have symptoms lasting beyond 3 months.

When you see a patient on postoperative day 1–2 after ACDF with new deltoid weakness, think C5 palsy before thinking hardware failure or hematoma. It characteristically appears at a mean of 1.7 days. Check preoperative imaging for cord rotation. Rotation ≥10° flags elevated risk.

Cervical hematoma demands a low threshold for return to the OR. Nearly half of all hematomas require surgical evacuation, and the window before airway compromise narrows quickly. Flag your highest-risk patients preoperatively: multilevel surgery, INR >1.2, BMI <24, and ASA ≥3 are independent predictors.

Esophageal perforation can present years or decades after surgery with sepsis, aspiration pneumonia, or even meningitis. When a patient with remote anterior cervical surgery presents with any of these findings, esophageal perforation belongs on the differential regardless of time elapsed since the index operation.
