Corrosion at the Head-Neck Taper as a Cause for Adverse Local Tissue Reactions after Total Hip Arthroplasty.

This retrospective case series describes 10 patients with metal-on-polyethylene THA who developed adverse local tissue reactions from corrosion at the modular femoral head-neck taper. Key Findings

  • Serum cobalt was elevated in all 10 patients and rose disproportionately higher than chromium (mean 9.55 vs. 1.39 ng/mL, p = 0.014) — the cobalt-over-chromium differential is the key diagnostic signal, not absolute elevation alone.
  • Plain radiographs were normal in all 10 patients at initial presentation, with no loosening, wear, or osteolysis — this diagnosis requires serum metal ion testing plus MRI or CT to detect fluid collections.
  • Harris Hip Score improved from 58.1 to 89.7 points at 13 months after revision (p = 0.01), with 8 of 10 patients gaining more than 20 points and 6 of 10 rated excellent (>90 points).
  • Serum cobalt fell from 10.96 to 1.61 ng/mL after revision (p < 0.05), while chromium and titanium did not change significantly — confirming the taper, not the bearing surface, as the metal source.
  • Abductor destruction was the most dangerous sequela, requiring constrained liners in 3 of 10 patients:
    –2 presented with gross abductor deficiency and recurrent dislocation
    –1 developed instability post-revision despite only moderate abductor necrosis
    –1 received a constrained liner prophylactically based on intraoperative abductor damage alone
Board PearlMetal-on-polyethylene THA can cause ALTR through taper corrosion — a disproportionately elevated serum cobalt over chromium is the diagnostic key.

Clinical Relevance

Taper corrosion was considered a historical concern largely solved by modern manufacturing — this paper proved it still causes clinically significant soft-tissue destruction, even with polyethylene bearings.

When a patient with any THA presents with unexplained hip pain, swelling, or instability and normal radiographs, check serum cobalt and chromium. A cobalt level disproportionately elevated above chromium should prompt MRI and consideration of taper corrosion as the diagnosis, regardless of bearing surface.

At revision, assess abductor integrity before committing to a liner choice. If significant muscle necrosis is present, use a constrained liner. The authors placed one prophylactically in their final case based solely on intraoperative findings, with no prior instability history.

A ceramic head on a titanium sleeve converts the interface away from cobalt alloy and avoids direct ceramic impaction onto a potentially damaged trunnion. This is the preferred reconstruction strategy described in this series.

  • Plain radiographs were normal in all 10 patients at initial presentation, with no loosening, wear, or osteolysis — this diagnosis requires serum metal ion testing plus MRI or CT to detect fluid collections.
  • Harris Hip Score improved from 58.1 to 89.7 points at 13 months after revision (p = 0.01), with 8 of 10 patients gaining more than 20 points and 6 of 10 rated excellent (>90 points).

Board PearlMetal-on-polyethylene THA can cause ALTR through taper corrosion — a disproportionately elevated serum cobalt over chromium is the diagnostic key.

Clinical Relevance

Taper corrosion was considered a historical concern largely solved by modern manufacturing — this paper proved it still causes clinically significant soft-tissue destruction, even with polyethylene bearings.

When a patient with any THA presents with unexplained hip pain, swelling, or instability and normal radiographs, check serum cobalt and chromium. A cobalt level disproportionately elevated above chromium should prompt MRI and consideration of taper corrosion as the diagnosis, regardless of bearing surface.

At revision, assess abductor integrity before committing to a liner choice. If significant muscle necrosis is present, use a constrained liner. The authors placed one prophylactically in their final case based solely on intraoperative findings, with no prior instability history.

A ceramic head on a titanium sleeve converts the interface away from cobalt alloy and avoids direct ceramic impaction onto a potentially damaged trunnion. This is the preferred reconstruction strategy described in this series.
