Surface Electromyographic Analysis of Exercises for the Trapezius and Serratus Anterior Muscles

Key Findings

  • The unilateral shoulder shrug drove the upper trapezius to 119% MVIC, significantly higher than every other exercise. This makes the shrug the clear choice when the goal is upper trapezius activation.
  • The prone overhead arm raise in line with the lower trapezius fibers maximally activated both the middle trapezius (101% MVIC) and lower trapezius (97% MVIC). One exercise covers both regions, which is efficient for programming.
  • Prone shoulder external rotation at 90° abduction activated the lower trapezius to 79% MVIC while keeping upper (20%) and middle (45%) trapezius low. This is the best exercise to isolate the lower trapezius, useful when you want to avoid upper trap dominance.
  • The serratus anterior was maximally recruited by the diagonal exercise (100% MVIC) and scapular plane abduction above 120° (96% MVIC). Both demand large amounts of scapular upward rotation.
  • Straight protraction exercises underperformed for the serratus: the dynamic hug hit only 53% MVIC and the shoulder press punch 62% MVIC. Upward rotation, not protraction, is the key driver of serratus activity.
  • Serratus activation was much higher above 120° (96% MVIC) than below 80° (62% MVIC), because most scapular upward rotation happens between 80° and 140° of elevation.
  • Scapular plane abduction above 120° co-activated the trapezius (49-79% MVIC) and serratus anterior (96% MVIC) simultaneously. This is the exercise of choice for retraining coordinated scapulohumeral rhythm.
Board PearlSerratus anterior is driven by scapular upward rotation, not protraction — abduction above 120° and diagonal exercises beat the punch and dynamic hug.

Clinical Relevance

When you prescribe scapular rehab, match the exercise to the muscle you want to load. The upper trapezius responds to the shrug, but the clinically underactive muscles in impingement (lower trapezius and serratus anterior) need targeted work.

The key mental model: serratus anterior activation tracks scapular upward rotation, not protraction. This is why the classic punch and dynamic hug underperform, while abduction above 120° and diagonal patterns excel.

In patients with impingement, who show increased trapezius but decreased serratus activity, the diagonal exercise is valuable because it loads the serratus while reducing trapezius contribution, and is often better tolerated than full abduction. Remember the study was done in healthy subjects, so extrapolate to painful shoulders with caution and respect the painful arc.

Clinical Relevance

When you prescribe scapular rehab, match the exercise to the muscle you want to load. The upper trapezius responds to the shrug, but the clinically underactive muscles in impingement (lower trapezius and serratus anterior) need targeted work.

The key mental model: serratus anterior activation tracks scapular upward rotation, not protraction. This is why the classic punch and dynamic hug underperform, while abduction above 120° and diagonal patterns excel.

In patients with impingement, who show increased trapezius but decreased serratus activity, the diagonal exercise is valuable because it loads the serratus while reducing trapezius contribution, and is often better tolerated than full abduction. Remember the study was done in healthy subjects, so extrapolate to painful shoulders with caution and respect the painful arc.
