Key Findings

  • Nonoperative treatment failed in 75% (9 of 12) of young patients, while arthroscopic Bankart repair failed in only 11.1% (1 of 9). Early surgery reversed the natural high recurrence rate of first-time dislocations in this age group.
  • Of the nine nonoperative failures, 6 (66.7%) went on to open Bankart reconstruction. Waiting for recurrence commits many patients to a larger salvage operation later.
  • Functional outcomes strongly favored surgery, with a SANE score of 88 vs 57 (P=0.002) and an L'Insalata score of 94 vs 73 (P=0.002).
  • Surgery did not cost motion: external rotation loss was 4° operative vs 3° nonoperative, not significantly different. Stabilization was achieved without stiffness.
  • All patients had both a Bankart lesion and a Hill-Sachs lesion on MRI and arthroscopy, confirming these as the essential structural injuries of traumatic anterior instability.
  • The single arthroscopic failure was a traumatic re-dislocation during football at 14 months, not a spontaneous failure of the repair. The repair held under normal loads.
  • No cases of reactive synovitis or stiffness from the bioabsorbable tack were seen, though the authors note this is a recognized complication of the Suretac device in other reports.
Board PearlEarly arthroscopic Bankart repair cuts recurrent instability from 75% to 11% after a first-time traumatic dislocation in young athletes.

Clinical Relevance

When a young athlete presents after a first-time traumatic anterior dislocation, the number that should drive your counseling is a 75% recurrence risk with nonoperative care. This trial shows early arthroscopic Bankart repair drops that risk to about 11%, with better functional scores and no loss of external rotation.

The cost of waiting is not neutral: two-thirds of nonoperative failures here needed open reconstruction, a bigger operation than an early arthroscopic repair.

Weigh the caveats. This is a small, single-sex military cohort with high physical demands, so absolute recurrence rates may overstate risk in a sedentary patient. The bioabsorbable tack is also of historical interest; modern practice uses suture anchors.

Still, the core lesson holds and is heavily tested: age and activity level drive recurrence, and early stabilization changes the natural history in the high-risk young athlete.
