Prospective cohort of 29 patients with massive rotator cuff tears (complete detachment of ≥2 tendons) repaired using a laboratory-validated transosseous technique. The study asks whether biomechanical optimization of the repair construct can reduce retear rates below the historical 50-70% and whether muscle atrophy and fatty degeneration are reversible after repair. Follow-up averaged 37 months with MRI-based structural assessment.
Every patient with a structurally intact repair in this series achieved excellent function. That single finding reframes how to counsel patients preoperatively: the goal of surgery is not just pain relief but achieving a durable structural repair, because functional recovery scales directly with repair integrity.
When evaluating a patient with a massive tear, check the acromiohumeral distance on a standing AP radiograph in neutral rotation. If it is less than 6 mm, the head is statically subluxed and direct repair carries a poor prognosis — these patients were excluded from this series and warrant a different operative strategy.
Fatty degeneration on preoperative MRI is a one-way street. It does not reverse after repair, and it progresses faster when repair fails. This is the mechanistic case for early operative intervention. Before Goutallier stage 3 or 4 changes develop. Rather than prolonged nonoperative management.
